In the mid game, stop chasing your opposing laner and start chasing objectives: dragon, towers, baron, enemy blue buff, enemy red buff and vision. This way, your team will be able to further expand the lead and will make really hard for the enemy team to make a come-back.
Champion kills? Killing a champion is not even an objective, more like champions are the hindrance to get the objectives.
Lost the laning phase? Group with your team mates or gank other lanes.
Won the laning phase? Follow your opposing laner wherever he/she goes.
In the mid game, always prioritize objectives over kills.
In the late game, this is all about team synchronization which is outside of your control.
